New Patient Information and Procedures

Schedule your annual exam for the week after your period.
Bring a list of all your current medications and supplements including dosages.
Bring your insurance card and photo ID with you.
Bring copies of your medical records to your appointment, especially abnormal pap test results.
If you are transferring from another practice, please make sure to Request Your Records.
All new patients need to fill out ALL of these forms: New Patient Demographics Registration, New Patient Form, Annual Consent Form and this HIPPA Consent Form prior to your appointment.
If you are scheduled for an annual gyn exam, please also fill out this form.
If you are under the age of 18, please fill out this New Pediatric Patient Demographic Registration.
